Although we collect fees at the time of service, we will gladly submit claims to your insurance company on your behalf. We firmly believe that dentistry should be a personalized experience, where the patient’s needs and the dentist’s expertise work perfectly together.

IMMEDIATE ACCESS: There’s no waiting period for any of your benefits. The moment you sign up, your coverage starts.

DISCLOSURES
RESIDENTS OF THE STATE OF WASHINGTON SHOULD BE AWARE THAT:
- The membership plan is not (i) insurance, (ii) a qualified health plan under the Affordable Care Act, or (iii) payment for access to discounted services. Rather, membership plans are between plan members and Providers for care provided in the first visit of the membership plan. Subsequent visits, discounts, and other services included in the membership plan are provided to plan members at no additional cost.
- The membership plan runs for 12 months. Plans will be renewed at the members subsequent appointment following their plans renewal date to avoid the potential for pre-payment of care.
- Payment for treatment not included in the membership plan is due at the time of service.
- The savings offered in the membership plan cannot be combined with any other offers or discounts.
- Membership plan payments are non-refundable.
- Value is calculated based on Elevate Family Dentistry’s standard professional fees
DENTAL INSURANCE VS MEMBERSHIP
PLAN WHAT’S THE DIFFERENCE?
Dental insurance is typically provided through an employer
or purchased individually. It comes with:
MONTHLY PREMIUMS: You pay regular monthly fees, regardless of whether you visit the dentist or not.
DEDUCTIBLES & COPAYS: You'll pay out-of-pocket until your deductible is met, and then cover part of the cost through copays.
LIMITED COVERAGE: Certain procedures, like cosmetic work, may not be covered at all, and there are annual maximums on benefits.
WAITING PERIODS: You may have to wait months before certain procedures are covered.
COMPLEX RULES: Insurance companies often require pre-approvals for treatments and impose coverage restrictions, making it harder to get the care you need when you need it.
